Majú nepatrne viac zdravia ako väčšina normálnych ľudí v hre a sú tiež výraznejšie rýchlejší a silnejší. Ale narozdiel od ľudí nemajú funkčnú nepriestrelnú vestu ani brnenie takže ich pomerne ľahko dostať s 2-3 strelami do hlavy s loveckej pušky. Majú 80 HP.
Zvyčajne nosia pri sebe Morfium, Rad-Away, Jet, malé množstvo zátok (4-8) a niekedy aj položku "ľudské mäso" ktoré nemá žiadnu hodnotu a ešte rôzne iné drobnosti.

Používa: perlíky (obojručné kladivá), útočné pušky, raketomety, rotačné guľomety. Majú viacej brnenia čo ich robí odolnejšími.
Kapitán supermutantov
Používajú: čínske útočné pušky, superkladivá, raketomety, rotačné guľomety. Ich brnenie je ladené do čiernej farby a majú ho najviac zo všetkých supermutantov.
Vládca supermutantov (Broken Steel)
Používajú: Najčastejšie asi trojpaprskové laserové pušky, superkladivá, laserové rotačné guľomety a zriedka rotačné guľomety. Na to akú majú palebnú silu a aký sú odolní za nich dostávate relatívne málo XP a nenosievajú ani moc výbavy. (Aj keď celkom často majú pri sebe Mini atomovku)
Perla: Raz pri vychádzaní z GNR sa mi podarilo zastihnúť skupinku zrovna respawnovaných supermutantou, keď som vchádzal dnu vonku totiž bola len jedna hliadka. No a ako som tak vyšiel von tento macher (Vládca supermutantou) sa objavil zrovna vedľa mňa, keďže som vtedy nebol moc silný, rýchle som mu s brokovncou vystrelil tú jeho trojpaprskovú laserovú pušku a rýchle som ju zodvihol zo zeme aby ju nemohol vzať on. Keďže tento bol odzbrojený a moc odolný na to aby som ho likvidoval nechal som ho a rýchlo som sa vrhol zachrániť tú jednu hliadku pred Kapitánom supermutantov ktorý sa tam objavil tiež, zastrelil som ho a samozrejme Vládca sa hneď vrhol po jeho čínskej útočnej puške, stihol ju zobrať a začal z nej po mne páliť ako Rambo (Držal ju v jednej ruke pod pažou). Túto pušku som mu tieť rýchle vystrelil z rúk a hneď na to sa on rozbehol do nedalekej zrúcanej školy kde boli další supermutanti (asi zhánať dalšie zbrane), mal som ho v pláne nechať ísť (nechcel som míňať náboje) ale keď sa zo školy v ktorej vlastne nebol nikto len supermutanti začali ozývať tie supermutantie výkriky rozbehol som sa pozrieť čo sa tam deje (tá hliadka z bratstva tiež). A čo nevidím cez zrúcané steny, tento zrádca (ten Vládca supermutantov) tam mláti holými rukami svojich vlastných (prve po nich hádzal granáty ktoré tam boli na poličke :D) Ja som z toho fakt nemohol :D. Chudák sa asi snažil získať nejaké zbrane :D. Nechal som ich nech sa bijú (1 Vládca VS asi 5 Supermutantov). Po chvíli to Vládca vzdal a rozutekal sa von tým druhým vchodom do školy (nie ten pri GNR). Tak som tam teda nabehol (aj s hliadkou) a dorazil tých supermutantov čo tam zostali a išiel som po stopách Vládcu. Natrafil som na ňho zhruba tam kde ste prvýkrát stretli Lyonsovú, Redin a Vargasa. Tu úž som ho dorazil ja osobne kedže po bitke s jeho súdruhmi mu zostalo celkom málo života a stále nemal žiadne zbrane :D.
Supermutant Behemoth
Zvyčajne používajú požiarny hydrant namontovaný na vodovodnom potrubí ako provizornú chladnú zbraň, tiež môžu používať svoje obrovské päste. Obe spôsobujú obrovské škody.
Obvykle ich zabijú dve strely z Tlsťocha, záleží na vašej skúsenosti s veľkými zbraňami. Majú 2000 HP.
